This must contain: Identification of the content and its location before removal. Your full name, mailing address, phone number, and email address. A sworn statement, under penalty of perjury, affirming that you believe the removal was made in error. A declaration consenting to legal proceedings and agreeing to accept service of legal documents. Your physical or electronic signature. We may reinstate the content after receiving a valid counter-notice unless the complainant initiates legal action.
